The World Health Organization has warned that the true scale of the Ebola outbreak in the Democratic Republic of Congo is likely significantly larger than official records indicate, with the actual caseload estimated at two to four times the figures currently reported.

Speaking on Tuesday, the UN health agency said the official count, which holds that the hemorrhagic fever has infected nearly 2,000 people since the outbreak was declared in mid-May, almost certainly understates the reality on the ground.

Why the gap between reported and actual cases

The WHO attributed the discrepancy in large part to a high number of deaths occurring outside medical facilities, where cases are believed to go unregistered. In conflict-affected and remote areas of eastern DRC, where access is limited and trust in health authorities can be low, this problem is particularly acute.

A familiar challenge in Ebola response

Underreporting has been a recurring issue in past Ebola outbreaks across Central and West Africa, where stigma, fear and logistical barriers have historically pushed families to care for sick relatives at home rather than seek treatment. The current outbreak is unfolding in areas where armed group activity and population displacement have further complicated the work of responders.

What happens next

WHO and its partners are expected to intensify surveillance efforts and work with local leaders to encourage patients to come forward for treatment. Greater transparency in case reporting will be critical in the coming weeks as the agency reassesses its estimates and adjusts its response strategy accordingly.
